Job Summary
The Health Promotion Coordinator is a professional employed by the Kanabec County Community Health. This position will provide leadership and support the work with the Better Together Coalition of Kanabec County to promote community health by promoting community health through outreach, planning, implementation and evaluation. The coordinator will also work to build new relationships with individuals, groups and other local organizations to further advance the mission of the coalition.
The coordinator works collaboratively with community partners such as schools, law enforcement, health systems, treatment and recovery programs, businesses, municipalities and others to increase protective factors to reduce the harm of alcohol, tobacco, and other drugs. This position will also participate in promoting mental health and wellness prevention efforts in Kanabec County.The position will help to assess, plan and implement prevention programming throughout Kanabec County with a focus on primary youth prevention. Primary responsibilities of this position include providing a comprehensive approach to creating change in Kanabec County towards substance use and promoting well-being; engaging the local community in coalition prevention goals; providing coordination, leadership and organizing community prevention activities; facilitating education on substance use and well being within the community. Ideal candidate will be able to facilitate large and small groups, develop and maintain positive relationships with community members and effective partnerships with schools, businesses and other organizations within the community.
Some Examples of Essential Duties
- Coordinates and administers assigned County health promotion programs and participates in the assessment, planning, administration and evaluation of various public health programs.
- Manages the budget for assigned health promotion grants.
- Assists in conducting needs assessments of health promotion programs.
- May recruit and supervise program volunteers and interns.
- Engages community leaders to foster participation and ownership to achieve prevention goals.
- Demonstrates the ability to convene and lead community meetings.
- Plans policy initiatives in collaboration with others in order to implement policy change.
- Works with community partners to develop system and policy changes that favorably influence the health of Kanabec County residents.
- Serves as liaison to community organizations. Educates and advocates for health promotion and public health activities.
- Serves as resource person to the general community and agency staff in the promotion of health.
- Conducts public relations programs to create awareness of existing health concerns and preventive issues.
- Attends workshops, meetings and training to be knowledgeable about health promotion and preventive issues.
- Provides leadership to task forces, coalitions, and groups in the local area who are addressing preventive health issues.
- Provides consultation to schools, worksites or groups to develop health promotion activities and wellness programs and policies.
- Serves as a consultant to the general community in locating appropriate resources that address local health problems.
- Works according to good safety practices as posted, instructed, discussed, and written in policy.
- Collaborates with administrator in identifying health issues, as well as inventory of community resources.
- Develops and provides presentations as requested.
- Coordinates and participates in community events as assigned.
- Responsible for writing grant applications for health promotion activities with consultation from supervisor.
- Documents and completes various reports related to grant programs.
- Develops expected outcomes related to program areas.
- Evaluates effectiveness of programs and implements changes as indicated.
- Manages grant budgets with assistance of supervisor.
- Develops community interest involvement in health promotion activities.
- Performs other work as required by the County Public Health Director.Kanabec
MINIMUM QUALIFICATIONS
Combination of education and experience will be considered
- Bachelor’s Degree in Education, Health Education, or Community Health
OR
- Graduate from Baccalaureate program for nursing
- Must be currently licensed to practice their profession, where applicable
- Two years preferred in professional field.
- Experiences in policy, systems and environmental change approaches to better health preferred.
- Valid Driver’s License
