Job Summary
This will be a full-time position working across both family health and adult health. Position will include family home visiting focused on prenatal/pregnant women and families with young children using an evidence-based curriculum. The position will also include the provision of care coordination and case management to members in the community in the nursing home or living independently in their own homes. The goal of care coordination and case management is to help develop and coordinate person centered plans with individuals, their families, and support network that allows the individual to improve their quality of life through prevention or management of chronic illness and connection to resources to help them meet their needs.

75% Specialized Health Services:
- Provides specialized health services to County residents by developing, implementing and evaluating specific program areas, such as the following, but not limited to: Intensive Family Home Visiting, Long-term Care Consultation, Child Teen Check –Up Screenings and PCA Assessments, WIC and Home Care.
- Assists in collecting data and evaluating outcomes for the purpose of program development and statistics reporting.
- Participates in educational presentations addressing program specific issues within the agency and provides health education services within the community.
- Ensures program compliance with regulatory standards by keeping abreast of pertinent statutory and policy changes.
- Coordinate services and provide professional nursing consultation to other health care providers and community groups by participating on planning teams, task forces, and advisory committees for the purpose of influencing and developing policy and procedures related to various community public health issues and develop programs responsive to community health care needs.
- Provide orientation and training to Public Health Nurses, Home Health Aides, Health Educators, and LPN’s in the area of case management techniques, current medical and nursing technologies, current public health trends, and community resources.
- Assure quality care, cost containment, and appropriate level of service by monitoring, and evaluating contracts with health care vendors and providing on-site evaluation and consultation to vendor-supplied caregivers.
- Recommends implementation of health care programs to the supervisor, Director, and funding sources by researching pertinent legislation, health care trends, and community needs; by interpreting results of data collections; identify service delivery strategies and resources; and prepare comprehensive reports for specialized health service areas.
- Provide direct professional nursing services and consultation to county residents by making assessments, teaching, counseling, and administering skilled nursing treatments.
- Facilitates a variety of interdisciplinary team meetings and client conferences.
- Provide case management of clients and overall care coordination, assuring that appropriate health services are provided and are completed within case budget.
- Develops and provides health education and consultation services to community members, clients and family members, worksites, and other professionals on a one-to-one and group basis through home visits, clinics, health promotion and community outreach activities, responding to requests for information by phone or in person; coordinates and conducts health fairs; develops written and oral educational materials and curricula; and teaches classes on topics related to public health issues, disease prevention and recognition, and safety and injury prevention; facilities at-risk groups on public health issues at various community sites.
- Collaborates and communicates with agencies such as schools, health care providers, housing agencies, Family Services, law enforcement, clergy, and others to advocate for individuals, families or community groups regarding community/case need.
- Observes, evaluates and interprets the emotional and environmental conditions of clients and families to provide the services and information needed.
- Develops and participates in outreach activities to the community for agency programs.
- Acts as preceptor for students to gain clinical experience necessary for completion of course work.
5% Supervision
- Demonstrates, teaches, plans and supervises paraprofessionals and other health personnel regarding specific client nursing care, community clinic, and other setting as appropriate per situation.
- Directs the work of others in special projects or those engaged in programs directed by this position.
- Participates in the evaluation of paraprofessionals and other health personnel.
10% Quality Assurance
- Participates in risk management and other quality assurance activities by:

- Manages client records by maintaining accurate and complete documentation according to standard nursing format and other record compliance issues.
- Contributes to the evaluation of specialized program policies, procedures and standards of care to insure compliance with State and Federal rules and regulations by:

10% Administrative Duties
- Provides input and assists in the evaluation of agency programs and policies.
- Contributes to the planning of agency goals and programs.
- Attends meetings, seminars and conferences to keep abreast of changes in policies, procedures and concerns of the public health nursing profession and as it relates to family Health.
- Represents the agency in contacts with other agencies and the community.
- Understands and utilizes Public Health and the Public Health Nursing philosophy and knowledge.
- Assists in the development of grants as assigned.
- Accountable for enforcing and following county safety policies.
- Responds to emergencies that affect the public’s health such as communicable disease outbreaks and nuisance problems as needed.
- Respond to Public Health emergencies as directed.
- Performs other related duties as assigned.
Qualifications:
- Current Minnesota licensure as a Registered Nurse
- BSN in nursing but would consider applicant who has a two-year degree but is working toward BSN (pay scale for four year listed, two year is less)
PREFERRED QUALIFICATIONS:
- Certification or eligibility for certification as a Public Health Nurse in Minnesota.
- One year of nursing or social work experience in home or community based services
